This tool runs entirely in your browser. Nothing is uploaded, nothing is stored, and you can even turn off your internet connection before you start.

How it works

  1. Set the starting date, and the dates fill themselves down the first column.
  2. Choose thirty rows for a month, or sixty if you measure morning and evening and want both on one sheet.
  3. The box underneath is for what you want to remember to ask. Rename it or remove it.
  4. Download and print. Keep the sheet where you keep the monitor, not where you keep the paperwork.

Frequently asked questions

Does it work out an average?
No, and deliberately. This is paper to write on, and a printed average that disagrees with the handwriting beside it is worse than none. Your doctor will want the individual readings anyway.
Can I have two readings a day on one line?
Yes — add columns. “Morning|2” and “Evening|2” with sub-readings written into each works for many people, though six narrow columns is about the limit of what stays legible in portrait.
What about blood sugar, or weight?
Change the column headings. The sheet does not know what the numbers mean, which is exactly why it can be any of these logs.
Is anything uploaded?
No. The PDF is drawn in your browser, so whatever you type stays on your computer.
Is it free?
Yes. The free plan adds a small line at the foot of the sheet; Pro leaves it off.
